Complete the form, including your National Insurance number, and sign and date it1.

Place the form on a hard, flat surface, holding your camera/phone directly over the form.

Take the photo, using a flash if necessary, making sure the photo captures all of the form, including the section in the top left that says our special codes.

Now send us the photo of your completed form using the secure upload form section below.

What you will need to return to us in the post

Please return the completed, original form to us as soon as you are able to, even if that is a few days or weeks down the line. If you have received an information pack from us, it should include a prepaid envelope. If you need a prepaid envelope, call 0800 009 3388 and we’ll pop one in the post to you.
